Change the two instances of "Journal-21" to the name(s) of your note and folder (if you're also using a folder).

Be sure the note name (and folder) already exist before running the shortcut.

After running the shortcut, text similar to this is added to the bottom the of the Apple Note (time date on one line, text on the next line):
